From the North
Taconic State Parkway South. Taconic State Parkway South becomes Bronx River Parkway. Turn right onto Chatterton Avenue. Take the first left onto Tarrytown Road/Route-119 East. Continue to follow Route-119 East. Turn right onto South Lexington Avenue. Turn left onto Maple Avenue.
From the South
Weaver Street/Route-125. Take the third left onto Heathcote Bypass. Turn left onto Secor Road/Route-125. Continue to follow Route-125. Turn left onto Maple Avenue.
From the East
I-287 West/Cross Westchester Expressway toward White Plains. Take the Westchester Avenue exit, Exit 8, toward Westchester Mall Place/White Plains. Turn slight left onto Westchester Avenue. Make a U-turn onto Route-22 South. Turn left onto Mamaroneck Avenue/Route-125. Take the first right onto Maple Avenue.
From the West
Saw Mill Parkway North/Saw Mill River Parkway North Merge onto I-87 North/New York State Thruway via Exit 20 toward I-287/Tappan Zee Bridge Merge onto I-287 East/Cross Westchester Expressway via Exit 8 toward White Plains/Rye Merge onto Route-119 South via Exit 5 toward White Plains. Turn right onto South Lexington Avenue; Turn left onto Maple Avenue.

When you come for your Appointments, please remember to bring the following
- Driver’s License or a valid ID
- Insurance information
- Referral Letter (if required)
- Reports, X-rays, MRIs, CT scans etc. and any other relevant information List of medications (if any)
To cancel an Appointment
Telephone the office during business hours and allow at least 1 days’ notice so that we can offer your appointment time to patients on our waiting list.
We recognize that your time is valuable, and we make every effort to run on time. Occasionally emergencies or patients require a little more time, and these cause scheduling delays beyond our control. We apologize if we keep you waiting.
